Common diagnoses were (inguinal) hernia (8.9% of all diagnoses), hydrocele (8.9%), malaria (8.3%), diarrhoea and vomiting (5.2%) and AIDS (4.4%).

 

Dilatative cardiomyopathy (leading to congestive heart failure) continues to be  common among males and we still think that it might warrant some investigation given its poor prognosis.

 

Leading causes of death were: AIDS (5 deaths), stroke (4), congestive heart failure (4) and pneumonia (3). Two deaths each were attributed to diarrhoea and vomiting, irreducible hernia with peritonitis, liver cirrhosis, anaemia, poisoning and malaria.

Common diagnoses among adult females were: Malaria (15% of all diagnoses), incomplete abortion (11.7%), diarrhoea and vomiting (8.7%), AIDS 5.7% and ovarian cyste 4.9%.

Seven patients died from AIDS, three from pulmonary TB. Two deaths were attributed to each of the following causes: stroke, malaria and congestive heart failure.

 

There were 1523 admissions in the children’s ward and 52 deaths. The bed occupancy rate was 70% (26 actual beds)

.

Common diagnoses among children were: Malaria (60% of all diagnoses), pneumonia (14.7%), anaemia (10.3%), diarrhoea and vomiting (7.9%) and fractures (1.5%). Especially children may of course have more than one diagnosis.

Not surprisingly malaria was also the main cause of death, 16/52 (30.1%) of the deaths were attributed to this cause. Anaemia (9 deaths), pneumonia (8), meningitis (4) and diarrhoea and vomiting (4) were further causes.

So altogether there were  936 deliveries at Lugala Lutheran Hospital in 2003 (2000: 939; 2001: 882; 2002: 757). Of these 156 (16.7%) had to be managed by C-section (2000: 18.4%; 2001: 17.2%; 2002: 16.9%).

Thus it has been possible to reverse the steady decline in deliveries seen in previous years. One reason for this change may well be that we now offer pregnant women to stay in the hospital for 50 TSH per day while waiting for the delivery.

Altogether 455 major operations were carried out in 2003 (404 in 2002).

 

Minor procedures totalled  1753 in 2003.

 

Two hundred and twenty five (225) patients attended our dental care unit in 2003, 262 teeth were extracted.

 

From January until December 2003 altogether 549 diagnostic X-rays were taken

 

Laboratory examinations:

 

In 2003 (sputum) specimens from 384 patients were examined for AFB (2002: 321; 2001: 114; 2000: 95). 29 specimens were found to be positive for AFB.

 

Altogether 634 (HIV negative) units of blood were given to patients in 2003 (508 in 2002). In addition 38 possible donors were found to be HIV positive (20 in 2002). This gives a prevalence rate of HIV infection among blood donors of 5.9% (3.9% in 2002).

 

In 2003 93 male patients (44 in 2002) and 120 female patients (70 in 2002) were found to be HIV positive.

 

During the same period of time the syphilis serology (the RPR test) was positive in 99 (out of 1028) individuals most of whom (529) were referred for screening from the antenatal clinic of the MCH department . The figures are similar to those of 2002.

 

The total number of stool examinations was 5677 (3836 in 2002).

 

Microscopic urine examination totalled 2947 in 2003 (1185 in 2002).

 

Altogether 9459 blood slides were examined for malaria parasites, 5764 were positive.  The haemoglobin level was checked 10934 times, the concentration was below 7 gm/dl in 1504 specimens (13.8%).

 

The ESR was tested 514 times, a WBC was requested 285 times. Eight sickle cell tests were positive.

 

The CSF of 64 patients was examined, there was evidence of meningitis in 10 of the specimens.

 

Infusion Unit:

 

A total of 11592 infusion bottles were prepared in 2003, this includes 734 bottles for blood transfusions.
