LMH is situated close to the main tarmac road between Dar es Salam and Arusha. From Dar and Arusha daily busses are coming and you should stop in Msambiazi about 320 km from Dar soon after Koroqwe town and also about 320km from Arusha soon after Mombo town. From there you can try to get local daladala to climb the last 14km of the road to the hospital. 

Also you can contact the hospital in advance and get the hospital car to pick you up. The scenery it breathtaking.
